Course Change Request Form

This form should be submitted by the deadlines indicated in your Canvas student orientation course. Refunds (if applicable) are determined by the date received. If requested changes result in a balance due, this must be paid in full before a change can be processed.

Please note that all requests for continued enrollment are subject to approval by the Pre-College Programs office and are contingent upon meeting program expectations and maintaining satisfactory academic performance.
